Submit a Claim - Claim Instructions

To qualify for a payment, you must submit a timely and valid Claim Form with the required supporting documentation. The Claim Form must be uploaded online such that it is received no later than January 24, 2022, or, if mailed, be postmarked no later than January 24, 2022.
